have a terminal medical condition and the application of
life-sustaining procedures would serve only to artificially
prolong the dying process.

What should I do with my document?
Keep the signed original with other important documents in a place available to family members or your Power of Attorney.
Give copies to your primary physician and family members overseeing your medical well-being. and let them know of your wishes. Communication with family is the best way to avoid any confusion in the future. No document is 100% effective.
Record your Living Will online with the State of Arizona.
The Arizona Advance Directive Registry was created in May 2004 by the Arizona State Legislature as a database for storing advance directives (Living Will, Medical Power of Attorney, and Mental Health Power of Attorney). The Arizona Secretary of State oversees Registry filings, its security, and operations.

State Bar's website, but it is advisable to have an attorney
review the preparation of all your legal documents and you will need your
signature notarized. Most attorneys will charge you upwards
of $25 for preparing and notarizing this simple document.
